Description

The post holder must have be an experienced cardiac nurse with extensive knowledge of cardiac anatomy, physiology and pathology of cardiac disease in order to provide a high level of patient assessment for the suitability of cardiac intervention or surgery.

The post holder will be responsible for the timely and effective week day MDT clinical coordination across South London in cardiology and cardiac surgery services, playing an essential part of the smooth running and efficiency of the daily MDT meetings.

The post holder will ensure that all relevant MDT administrative tasks including allocating referrals, case preparation, documentation, follow up of administrative actions from the MDT meetings and data collection are completed in a timely manner and ensure all patients are discussed at the appropriate MDT meeting and facilitate or action the outcomes required from the clinical decision making.

By working closely with a wide range of clinical and managerial staff across South London, and providing a high standard of administrative support, the post holder will ensure patients receive treatment as soon as possible and ensure national targets are met.

Ensure effective theatre utilisation, minimise surgery cancellations and optimise pre-operative pathways.

The duties and responsibilities listed below are representative of the MDT clinical coordinator role and its purpose within the service:

they are not exhaustive and the post holder may be required to undertake additional or alternative administrative and managerial duties commensurate with the level of this post, to ensure the optimal efficiency of the service with oversight of all the units and their operating and procedural lists.

Clinical Responsibilities

  • Review all new referrals made on the MDT referral system
  • Clinical assessment of the cardiac disease, pathology and physiology in each referral, using advanced clinical assessment skills and knowledge.
  • Ensure all patients have undergone appropriate diagnostics
  • Using clinical knowledge request outstanding diagnostics from referring centres to allow a full MDT discussion regarding the patient's treatment plan
  • Review results and diagnostics and as appropriate outside of the MDT discuss with a clinician
  • Ensure patients' treatment is not delayed and is based on both chronological referral and clinical urgency by assessing each case referred to the cardiac tertiary centres.
  • Provide expert advice to referring centres regarding individual patients and their cardiac disease when required. If required to also speak with patients and carers to explain the referral and decision making processes regarding their cardiac disease.
  • Implement specialised and efficient pathways with both the referral and treatment centres, using indepth knowledge of the progression of cardiac disease and the treatment modalities available.
Operational & Administrative responsibilities - including systems and data management

  • Manage the MDT referral system across South London and allocate referrals to the appropriate week day MDT
  • Update the local trackers, ensuring that all appropriate patients are included in the MDT.
  • Produce a list of all new patients to be discussed and distribute to all members prior to the MDT discussion
  • Invite referring clinicians from the network to present and discuss their cases
  • Identify current patient care and/or treatment progress using the various patient record systems, in line with Trust and local policies, and contact referring clinicians when this is not available.
