An exciting opportunity has arisen for experienced individuals to join our Integrated Discharge Team.

The positions are an ideal opportunity for Nurses, Social workers, Allied Health Professionals, Nurse Associate and Therapy / Social care practitioners to shape this new service for our patients.


Shropshire Community Health Trust is offering an exciting opportunity for motivated individuals to work within the Integrated Health and Social Care Discharge Team at the Shrewsbury and Telford Hospital Trust.

The team is expanding and this is an ideal opportunity to be involved in the transformation of the service and work with the existing skilled team in the specialist field of Complex Discharge.


The post holders will work within the integrated team discharge providing a community focused approach to discharge planning and assessment, embracing the principles of a Discharge to Assess Model and providing a person-centred approach to care.

The role and service offer integration and partnership working with team managers, nurses, therapists, health and social care coordinators across both organisations to ensure a seamless service
