Complex Care Nurse (RGN)

About us:


Precedo Healthcare Services is a CQC registered provider of continuing healthcare, complex care and end of life care in the home.

We are commissioned by Integrated Care Boards (ICBs) and Local Authorities to care for and support people in the community.


We are looking to hire a full time Complex Care Nurse (RGN) to work across the West Yorkshire region, based out of our office in Brighouse, Calderdale.

Primarily Mon-Fri working, but this role will require some weekend work (planned in advance) in orderto accommodate our clients' needs and schedules.


Our service supports adults who live in their own homes and have complex, lifelong and often life-limiting conditions such as; Dementia, Muscular Dystrophy, Spinal Injury, Multiple Sclerosis, Autism, Cancer, Learning Disabilities/Difficulties, Epilepsy andAcquired Brain Injury.


The Complex Care Nurse Role:


  • Participate in clinical assessments for newly referred and existing clients. Undertake and advise regarding environmental risk assessments and other risk assessments relevant to each client and their individual situation;
  • Conduct regular, cyclic client reviews and respond accordingly to concerns and changes in client needs;
  • Produce, maintain, review and amend clinical care plans to agreed deadlines, for each client's team of care workers to follow;
  • Conduct regular reviews of care notes and clinical paperwork completed by care staff and give feedback;
  • Assist the Manager with monthly action planning for clients;
  • Provide training and guidance to care staff in Delegated Healthcare Activities such as tracheostomy care, ventilation care, cough assist support, manual evacuation, enteral feeding, medication administration, oxygen therapy, inhaler / nebuliser therapy,Diabetes care, suctioning. Etc. And/or liaise with the applicable commissioning bodies to arrange the necessary training for care workers;
  • Monitor and sign off care worker clinical skills and knowledge competencies;
  • Support the Manager to ensure that all client's care needs are met, through the allocation of suitably trained and skilled care staff;
  • Work effectively and in partnership with parents/NOK, advocates, GPs and other health professionals to contribute to high standards of care and to achieving client outcomes;
  • Build lasting professional relationships, attend and participate in multidisciplinary meetings in relation to client care.
  • Work alongside and in support of the Manager. Covering agreed management responsibilities during times of annual leave / absence;

The Successful Nurse Will:

Have experience working with adults with complex care needs, an active NMC pin (RGN) and a full UK driving licence;

  • Have experience of undertaking clinical assessments, risk assessments and producing clinical care plans;
  • Feel confident in their ability to review and assess staff competency and to deliver training & support to care workers, in order to develop staff knowledge, skills and understanding;
  • Have flexibility with working days and times, in order to best accommodate and support our clients and their care teams;
  • Be proactive in conducting their own research and learning on any unfamiliar health conditions. Be willing to attend agreed training sessions (in working time) in order to maintain and develop their knowledge and skills;
  • Maintain an up to date working knowledge of the NMC Code, CQC guidelines and industry best practice;
  • Follow and promote our company
