Description

We currently have an exciting opportunity for a Band 4 Nursing Associate to join the community nursing team in Chesterfield.

This role will work directly alongside the Ashgate District Nursing Team providing care for housebound patients in the community.

As a Nurse Associate you will be expected to provide community based individualised care to include long-term condition management, venepuncture and medication administration as well as completing various nursing assessments and following treatment regimes.

You will be based within the DN team, and you will provide nursing care to patients in their own homes within a designated area.
