Salary:
£31.76 per hour


Flintshire County Council is seeking a dedicated and experienced Social Worker to join our Learning Disability Service on a temporary basis to cover a vacancy.

As a vital member of our team, you will play a crucial role in supporting individuals with learning disabilities to live fulfilling and independent lives.


Key Responsibilities:
Provide high-quality social work assessments, support plans, and interventions for individuals with learning disabilities.

Work collaboratively with service users, their families, and other professionals to develop person-centered care plans.

Advocate for the rights and needs of individuals with learning disabilities, ensuring their voices are heard and respected.

Conduct reviews and evaluations of support plans to ensure they remain relevant and effective.

Liaise with external agencies and organizations to coordinate holistic support services for service users.


Requirements:

Registered with Social Care Wales.

Significant experience working with individuals with learning disabilities.

Excellent assessment and communication skills.

Ability to work effectively within a multi-disciplinary team.

Commitment to promoting the rights and well-being of individuals with learning disabilities.
