- Part Time position
- Various shifts available between 8am and 8pm Monday to Sunday

Job Summary
An exciting opportunity has arisen for Administrators and Coordinators to join the Surrey Heath Primary Care Network supporting local GP Practices.
The Patient Services Coordinator supports GP practices with ingoing and outgoing phone calls to patients, supporting access to services and continuity of care by helping patients interact and engage with the right support, at the right time, with the right health care professional.
Duties & responsibilities:
- Work in a telephony/ reception capacity taking patient calls and enquiries
- Provide coordination and navigation for people and their carers across health care services
- Working closely with social prescribers. Health and wellbeing coaches and other primary care professionals
- Direct patients to the right support or appointment type to best meet their needs
- Help people to manage their needs through answering queries, making and managing appointments
- Provide good quality written and verbal information to help patients make choices about care
- Support the practice with the Patient Participation Group (PPG)
- Capture key information to enable comprehensive and accurate records of support
- Inputting key information into clinical systems and adhering to data protection legislation
- 4 GSCE's (C and above) or equivalent.
- Experience of working with the NHS or medical environments
- Experience coordinating with multiple stakeholders or individuals to meet specified outcomes
- Experience providing advice and signposting
- Experience of using EMIS Web or a patient clinical system
- Knowledge and experience of working with Microsoft Office packages (PowerPoint, Word, Excel)
- Knowledge of safeguarding interventions and awareness of the Mental Capacity Act
- Able to demonstrate experience of effective planning and organisations skills to deliver targets and deadlines
- Able to analyse and interpret information and present results in a clear and concise manner
- Excellent verbal, written communication skills.
- Ability to work independently and manage own workload
- Ability to build strong professional relationships
- Good customer care skills.
- Enjoy working as part of a team and willing to help and assist others where possible.
- Understand the importance of strict confidentiality when dealing with any patient details and company information.
