An exciting opportunity is available to join the Clinical Operations Teams as a Capacity Co-ordinator.

You will be forward thinking, dynamic and highly motivated and comfortable both supporting and challenging others in equal measure.

You will be a key player in the operational running of the Trust working alongside the Clinical Site Managers to provide a seamless 24/7, 365 days service.


Individuals will need to be organised, possess excellent interpersonal skills and the ability to work with competing pressures and priorities which will frequently change.

This role will involve working closely with clinical ward areas across the Trust and the wider MDT involved with the safe, effective and timely provision of care on the wards and departments during the day, weekends and at night.

You will work closely with other senior nurse bleep holders and Medical teams.

  • Support the Clinical Operations and Patient Flow functions and the Wards to identify complex discharges on admission.
  • Provide accurate and timely information relating to patient transfers and discharge, maintaining awareness of the overall 'pressure' profile of the Trust for communication to the Patient Flow Sisters / Charge Nurses and Clinical Duty Managers
  • Champion the usage of, and staff compliance with, trust bed capacity management and escalation tools, discharge processes and principles.
  • Proactively liaise with wards to ensure that letters A are routinely given to relevant patients and relatives on admission
  • Monitor medical outlier listings to facilitate medical reviews and ensure appropriate action taken, escalating when necessary.
  • Provide up to date information on discharge for individual patients and any foreseen delays for the multidisciplinary team meetings.
  • Communicate effectively with all members of the Multidisciplinary Team and other agencies, including Consultants, therapists, nurses, social care teams, other NHS, commercial and voluntary organisations. Supporting and promoting teamwork generally through effective dissemination of information.
  • Update electronic systems as required e.g. MEDWAY and external escalation reporting systems within agreed timescales.
  • Provide accurate capacity and demand data for operational and/or tracking meets as required.
  • Actively participate in Board rounds on a regular basis to link with Ward managers / designated ward coordinators in planning discharge. Escalate identified issues as appropriate.
  • Provide accurate and timely information for ratification of delayed discharge list, conference calls, allocation and tracking meetings.
  • Track patients and ensure all administrative work completed for patients identified as meeting the criteria for NHS Continuing Healthcare funding.
  • Keep up to date with legislation and processes that may have an influence on the outcome for transfer of care/discharge of all client groups
  • Contribute towards the development of a strategy for improvement in areas where hospital discharge fails to reach acceptable standards.
  • Participate in investigations of complaints, as required, which specifically relate to a transfer of care/discharge process.
  • Maintain accurate and contemporaneous records relating to individual practice and patient discharge.
  • Adhere to the Trusts Adult Bed Management Policy
  • Effectively monitor patient progress towards Expected Date of Discharge taking action to ensure that delays are promptly addressed by the relevant ward or department.
  • Promote the corporate image of the trust to all groups and organisations both within the trust and external stakeholders.
  • Maintain confidentiality and manage information sensitively. Demonstrate discretion and respect when communicating with patients, colleagues, and other agencies.
  • Monitor requirement for equipment, including oxygen, required on discharge. Liaising with appropriate agencies to ensure availability for date of discharge.

EDUCATION

  • Ensure that staff are informed and updated on changes to process, documentation, or pathways for transfer of care/discharge
  • Ensure own education and development is in line with the requirements of the role
