Description

The post of Senior Discharge Practitioner will require an experienced Social Worker to work between the psychiatric wards at City and Hackney Centre for Mental Health and the Community Mental Health Teams in Hackney providing an effective and efficient, person centred hospital discharge service.

To participate in the provision of effective assessment for individuals with mental health problems inpatient on the psychiatric wards and developing robust discharge plans into the community in line with the Care Act, MHA MCA and HRA.

To provide professional, managerial, and clinical supervision to supervises providing support worker input to the discharge pathway.

To contribute to the development of practice within the discharge pathway, providing links with in
- patients and other statutory and non statutory organisations.

To provide advice and support to the psychiatric wards working with those in mental health crisis and facing a range of social care issues.
