Description
The post holder will provide expert clinical knowledge and interventions, advice, support and to the rough sleeper task force. To undertake rapid assessment and support to adults 18+ who are homeless or rough sleeping.

To provide specialist mental health assessment and support to adults who are housed and supported by the local authority but whose tenancy is in jeopardy due to a decline in their mental health.

To participate in the rough sleeper task force/mental health alliance multi-agency meetings, to participate in multidisciplinary, multi agency meetings.

  • Take responsibility for the specialist mental health assessment of a complex cohort, which includes, specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans.
  • To be a key member of the rough sleeper taskforce, providing appropriate advice to the rough sleeper task force, local authority staff, third sector staff, multidisciplinary and Management Groups.
  • To monitor and evaluate practice based on contemporary and current trends and initiate changes in the light of these findings.
  • To promote, encourage and develop integration between MPFT, local authorities and third sector organisations.
  • Facilitate effective early assessments for individuals identified through the rough sleeper task force or referred from housing or third sector colleagues.
  • To aid discharge planning from acute inpatient units and local hospitals PRH and RSH
