Description

We have the opportunity for a permanent Complex Discharge Practitioner (CDP) to join the Complex Discharge Team (CDT) at UHS.

We are seeking a patient focused and highly motivated Registered Nurse, Physiotherapist or Occupational Therapist to join us.


The successful applicant will be responsible for guiding patients and their families through the discharge process by completing assessments, supporting wards, gathering clinical evidence and coordinating with different local authorities.

The role is full time with working hours from 8-4pm. It will be expected that you will work some weekends.
The CDT works across the Trust. We facilitate the timely and efficient discharge of patient with complex needs with safety always as our priority.

As a CDP you will be on the wards, using your clinical reasoning skills to understand the patient's clinical presentation, prognosis and discharge pathway.
