Noble Tree Foundation are looking for an enthusiastic and passionate individual to support their service with compassion and professionalism ensuring that the people who use our service are kept at the center of all we do.


Duties:


  • Support Planning and Risk Assessing.
  • Carry out new service user welcomes, inducting service users into the service including registering with doctors, etc.
  • Ensure that clients have a full understanding of their rights and responsibilities.
  • Support clients to articulate and achieve their goals and targets as defined/ agreed within their support plans and support contracts, offering personal, holistic, and emotional support.
  • Hold planned keywork sessions each week with clients using a personcentred flexible approach.
  • Offer a creative but assertive approach in order to seek to reengage rather than taking the route of punitive action that may lead to disengagement and eviction
  • Regularly review and develop mutually agreed support plans with clients and any other professional involved in the support of those individuals e.g. Key Workers, Probation Officers, social workers and CPN's.
  • Hold multi agency meetings, coordinating the support of the client with other professionals
  • Update records within all client documentation, accurately and timely.
  • Provide support to help clients manage their own lives and their home to their full potential including dealing with benefits, budgeting and neighbour relations.
  • Motivate and challenge clients so they realise their goals.
  • Liaise between clients and external agencies regarding the provision of specialised support when appropriate.
  • Deal promptly with all complaints and protection from abuse disclosures.
  • Provide clear, accurate and good quality information in respect of all clients in line with agreed procedures ensuring that appropriate information is shared with the line managers.
  • To deal with the immediate support needs of clients as appropriate, providing crisis intervention where necessary
  • Assist with securing moveon to appropriate accommodation.

Qualifications and experience:


  • Proven experience of working with people who have complex needs
  • Must have experience working with people or in settings providing services for domestic violence, Substance and Alcohol Misuse, Challenging Behaviour, Prison Leavers, and Mental Health issues.
  • Experience working in a Supported Housing environment, with experience of void and arrears management and dealing with housing benefit claims.
  • Willingness to learn and develop new skills, open to changing needs and growth of the business
  • Creative thinker and proactive problem solver ability to resolve incidents calmly and productively
  • Demonstrate a great positive attitude, high levels of energy, drive, enthusiasm with a passion for learning, and being an active and supportive member of a dynamic team
  • Emotional intelligence, demonstrate selfawareness, selfregulation, motivation, empathy, Self-Reliance, and social skills.

Job Types:
Full-time, Zero hours contract


Salary:
£12.00 per hour
