HOURS: 37.5 per week


SALARY:
Band 8a (£55,903 - £61,996 based on experience)

The Role:


The post-holder is expected to be an experienced digital lead with considerable transformation experience, preferably within a primary care environment.

The post-holder will help practices and the
PCN adopt new technology and other initiatives to improve access to care, enable practices and PCN staff to work more effectively, coordinate across digital platforms and critically enable patients to self
- serve.


The post holder will work with the Clinical Director, Practice Managers and the leadership within a Primary Care Network (PCN) to create a vision to guide service improvements, identify opportunities to make tangible positive transformation, and provide the inspiration, expertise and leadership to deliver changes in a collaborative way.

This role will include delivery of a combination of the following responsibilities:
Improve adoption and/or optimisation of new technology to enhance patient access and experience and increase PCN productivity.


Build relationships and facilitate collaboration between practices and the wider system to support the delivery of care to patients (including shared appointments between practices to aid delivery of enhanced access).


Lead an improvement approach to change including building capability for quality improvement within the PCN and system wide approaches to problem solving.

Review and improve the PCN's digital maturity.

Use data, and improve data quality, to:

  • Understand demand, capacity and activity and drive improvements in:
  • Patient experience of access.
  • Operational efficiency including better matching capacity to need.
  • Staff experience at work.
  • Support population health management.


The post holder will work closely with staff working at practices, such as practice managers and staff at the PCN to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered.


Experience of working in general practice with a good working knowledge of general practice systems (including IT, digital and BI tools), processes, and procedures; or experience of successfully delivering change and improvement programmes in a patient (or customer) facing environment.


A track record of communicating and engaging with a wide range of staff and stakeholders; building excellent and trusted relationships.


Experience of using programme management and improvement techniques to drive change forward, including translating strategic objectives into tangible plans for delivery and following these through to benefits realisation.


Ability to extract, collate, visualise and use data and insight to both drive change, and to understand and adapt programmes of work while in progress to allow learning to be integrated into delivery.


Knowledge of Quality Improvement tools or a willingness to engage in the Quality Improvement training offers available to support the Digital and Transformation Lead role.

Ability to understand, interpret and present complex and/or sensitive information


Ability to interpret qualitative and quantitative information from a range of sources and present it in a clear and concise way to stakeholders.

Excellent communication skills and an ability to engage successfully with a wide range of people at all levels within an organisation


Experience of championing diversity and inclusion and promoting actions to make improvements to the experience of diverse groups, including promoting digital inclusion.

An ongoing commitment to improving NHS services for the benefit of patients.


About North 1 PCN and Background


The North 1 Primary Care Network (PCN) is a group of 5 independent GP practices in the centre of Islington, covering a population of around 53,000 residents.

The PCN is brought together by NHS England's PCN DES contract to deliver against its specifications and other tasks.

The PCN works with multiple stakeholders as part of the North Locality, a multi-agency group focused on identifying population health needs and identifying streams of work to support these.

This is part of Islington's
'Fairer Together' borough partnership.

The population within the PCN geography is diverse, often with significant and complex health and wellbeing needs.
