The CLDT Speech and Language Therapy Service provides communication and dysphagia support for those adults with LD with complex physical and mental health needs.

The team supports the adults with learning disability and works closely with the person's family and carers at home and day opportunities settings across a range of provider organisations to maximise potential and ensure optimal health and wellbeing for the person.


This is a developing specialist band 5 post with clinical responsibility for assessing, diagnosing and managing identified communication needs for adults with learning disability who are eligible for the CLDT.

It is a community role within a specialist MDT and the post offers the opportunity for active contribution to team working.

There are opportunities to develop dysphagia competencies within the role.

Job Description:


  • Key Responsibilities/Duties
1

Risk assess, screen, assess, diagnose and manage the identified communication needs of adults with learning disability with complex health and behavioural needs who are eligible for support from the South Staffordshire Community Learning Disability Team (CLDT)

Work closely with the person with learning disability to help them to understand Speech and Language Therapy input and to support decision making in relation to consent to SLT in line with the Mental Capacity Act - 2005

Advise and support the person, their family/carers in their understanding of any communication needs identified and the decisions made relating to options that are offered

Work closely with Speech and Language Therapy colleagues in planning and delivering a service to meet the identified needs of those referred to the service

Work with relevant medical, nursing, AHP, Psychology, social, educational professionals and carers; either directly or indirectly, to ensure that they understand a person's communication profile and the best way to support them is identified and agreed

Provide written programmes of work and reports for those on the post holder's small case load

To represent the service by attending meetings in relation to individuals on the case load

Attend relevant in service training courses and Trust statutory mandatory training as well as relevant and agreed external training and other CPD opportunities to develop the specialist skills, knowledge, techniques and strategies relevant to supporting this population and in particular for communication and dysphagia

Attend Speech and Language Therapy and AHP Meetings, Team Meetings, Clinical Excellence Groups, MDT meetings about individuals with learning disability as appropriate

Provide statistical and other information required by the Team Leader and Trust

Maintain up to date and accurate case notes in line with professional and Trust standards and policies

To be aware of and follow Trust polices and departmental guidelines when carrying out work in the organisation.

Be responsible for own personal and continued professional development and identify objectives and training needs via supervision and an annual professional development conversation

To actively participate in supervision offered by the Trust

14

To alert the Team Leader to any incidents, risks or issues while carrying out work
