Description

This is an exciting role within the Discharge Planning Team where you will support the team and to provide the best service to our patients and colleagues.

You will provide a essential administrative service to the team to support patient care and discharge processes.

Contribute to the department, ensuring that it operates to the highest standards, for discharge planning.

Provide administration support for the Head of Discharge Planning to ensure smooth running of the department.

Act as a key to external organisations with regard to queries related to team activity

Be responsible for ensuring that all duties are carried out in accordance with the correct procedures and policies.

Manage mandatory training records and support the team in developing the range of competencies required.

Support workloads within the department and provide limited cover for colleagues in their absence.

Support Senior Discharge lead in management of Annual Leave requestsBook MPAs and 1 to1s for senior staff.

Manage personal files administrative e

Provide administration support at meetings as required

Deal sensitively and effectively with telephone enquiries from colleagues, ensuring confidentiality is maintained at all times.


Act as a link for the discharge planning team to ensure that the appropriate level of knowledge is acquired to support the administrative processes required.

And to liaise with Social Services, health providers and organisations providing streamlined patient
