Contract:
Permanent**
Hours of work:Full Time 40 hours per week, Monday to Friday 09.00 to 17.00

Salary:

Negotiable depending on qualifications and experience.**
Job Role:The role of the Placement Officer is to participate as a full member of the Placement Team for identifying and advising potential care home matches for screened referrals and enquires.

To work as a part of a team to identify suitable homes within Cedar Care Homes that will best meet the needs of the potential Resident.


Key Performance Indicators.

  • Screen incoming referrals and enquiries.
  • Identify and advise potential care home matches for screened referrals and enquiries.
  • Arrange, coordinate and confirm viewing schedules between enquirer and the home, in line with current government COVID19 guidelines.
  • Arrange, coordinate and negotiate funding in line with agreed Cedar Care placement costs.
  • Complete robust phone or face to face assessments for screened referrals and enquiries for potential admission to the homes
  • Liaise effectively with Placement team, Home, Clinical and Area managers to discuss how a potential resident needs and risks can be met.
  • Attend preadmission interim MDT meetings via video calls or face to face discussions before accepting placement if required.
  • Source required necessary equipment prior admission of a potential resident where appropriate.
  • Effectively communicating with the Placement Manager any potential concerns and issues relating to the placement of an individual.
  • Work in strict confidentiality when dealing with placement matters.
  • Create and give reports on allocated referrals and assessments.
  • Understand Cedar Care safe admission policies and procedures.
