Job Purpose - Care Coordinator
This role is pivotal in ensuring all patients receive the best possible care and service.

The job holder will support the practice to help support patients who may need more proactive help with non-medical issues, or simply with negotiating the complex web of services within health and social care.

The job holder will need to have a keen eye for details and will support the Operations Manager in ensuring clinical rotas are available.


Key Responsibilities

The Care Coordinator will:


  • Work closely with the Operations Manager
  • Complete a 'Personalised Care Institute' accredited care coordination course as well as mandatory online training modules.
  • Proactively support call and recall processes, encouraging and supporting patients to attend
  • Support with and undertake the call and recall for vaccination campaigns to ensure the highest possible uptake e.g. COVID, flu.
  • Work proactively with the pharmacy team to support medication reviews and regular drug monitoring requirements.
  • Coordinate the booking home visits and in house appointments, ensuring patients who need health reviews are seen in a timely manner.
  • Proactively support multidisciplinary, practice clinical or safeguarding team meetings (dates, agenda, papers) where required.
  • Ensure all coordinated activity is documented and coded accurately.
  • Undertake care planning where relevant, and felt to be beneficial to the patient. This can be as simple as a document specifying what specific issues are important to the patient, their goals, and any support offered to reach them.
  • Help patients to manage their needs through answering queries, making and managing appointments, and ensuring that patients have good quality written or verbal information to help them make choices about their care.
  • Provide coordination and navigation for patients and their carers across health and care services, working closely with Social Prescribing Link Workers, Health and Wellbeing Coaches (if applicable) and other primary care professionals.
  • Ensure clinical rotas are keyed onto the IT system for 6 weeks in advance, managing clinical meetings, appointments and appraisals to ensure sufficient time is allocated for attendance,

Key Working Relationships
Teamwork is essential in multi-disciplinary environments.

Effective communication is essential and all staff must ensure they communicate in a manner which enables the sharing of information in an appropriate manner.


  • GPs, nursing teams and other health professionals in practices
  • YHN staff
  • Operations Manager
  • Secondary care teams
  • MDT with communitybased teams e.g. HARA and Community Nursing teams

Post-holders will:

  • Undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities.
  • Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.
  • Review progress regularly and develop clear plans to achieve results within priorities set by others.

Pay:
From £11.44 per hour

Expected hours: 37.5 per week
