To support the Multi-disciplinary team within the primary care network to deliver effective, coordinated care for people with learning disabilities and their family carers, including Annual Health Checks, screening, vaccination programmes and the management of long term health conditions with the aim of reducing health inequalities.

To improve the lives of people with learning disabilities by minimising the impact of their condition through the delivery of excellent services to promote recovery and well being.

Provide coordination and navigation for people and their carers across health and care services, working closely with social prescribers, health and well-being coaches and other professionals.
