SALARY:
Agenda for Change Band 4 (£23,949 - £26,282 per annum)


HOURS: 37.5


BASE:
Nottingham West PCN staff base is at PICS Unit H4 Ash tree Court, Nottingham NG8 6PY.

The Primary Care Network (PCN) covers 12 GP surgeries and patients who live in the Beeston, Eastwood and Stapleford neighbourhoods.

The PICS Nottingham West PCN Social Prescribing team's ethos is to work with people in a person-centred way by focusing on 'what matters to you'.

We do this by offering non-clinical support to reduce isolation and loneliness, increase activity, reduce low level mental health difficulties such as anxiety, and improve people's social situations such as financial stability.

Social Prescribers are also connected to what's going on in the local communities and work closely with the voluntary sector and local groups to improve social networks for the people they work with and support community development in their area.


Social Prescribers use coaching techniques to develop a person's motivation and confidence to make positive changes around their own health and wellbeing.

They do this by offering a person-centred approach, and using skills such as motivational interviewing, coaching and resilience building and empowerment to allow people to have more control and choice in their lives.

ABOUT THE ROLE


Social prescribing empowers people to take control of their health and wellbeing to give time, focus on 'what matters to you' and take a holistic approach, connecting people to community groups and statutory services for practical and emotional support.

Link Workers support existing groups to be accessible and sustainable and help people to start new community groups, working collaboratively with all local partners.


Social prescribing can help to strengthen community resilience and personal resilience and reduces health inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity, by increasing people's active involvement with their local communities.

It particularly works for people with long-term conditions (including support for mental health), for people who are lonely or isolated, or have complex social needs which affect their wellbeing.


There are opportunities within the social prescribing team to work within specialist areas of interest, which will also compliment working alongside the PCN clinical teams.


Specific areas the social prescribing team are currently working within and are keen to hear from anyone that has an interest/experience in are; children/young adults, learning disabilities, mental health and dementia.


KEY RESPONSIBILITIES

  • To manage and prioritise own workload when taking referrals from a wide range of agencies
  • To develop trusting relationships by taking a holistic approach to patients
  • To work with local partners to ensure that local VCSE organisations and community groups are sustainable

KEY REQUIREMENTS

  • NVQ Level 3, Advanced Level or equivalent qualifications
  • Demonstrable commitment to professional and personal development
  • Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement

Nottingham West PCN is a group of GP practices (12) that are committed to work together to add additional services and roles to support the workload in GP practices.

We cover 108,865 patients within our locality.

Our network works closely with all practices and other key members of the network including pharmacies, Citizens Advice, Voluntary Sector and local district and borough councils.

Our aim is to continually improve services to patients utilising all available resources.
