A rare and exciting opportunity has arisen in the Joint Community Rehab (JCR) Team If you have strong organisational, verbal and written skills and would enjoy helping your team - get ready to show off those skills and apply

The JCR team help people to:

  • remain in their own home and maximise their independence
- avoid going into hospital unnecessarily
- become independent as soon as possible after a stay in hospital


We are looking for someone that is able to create and maintain the electronic rota for Support Workers appointments for client reablement programme to support clients gain independence to remain at home.

Responding to daily fluctuations to the rota, allocating appointments and last minute changes in the most cost effective way.

  • Excellent problem solving and decision making skills.
  • Great communication skills
  • The ability to plan own workload and respond to issues independently.
  • The ability to effectively organise own and team workload and meet deadlines.
  • Strong ICT skills to be able to minute and produce accurate records of meetings.
  • The ability to exercise discretion in dealing with sensitive information to maintain strict confidentiality where appropriate.
  • Excellent communication skills to deal with challenging and distressed clients and staff on the telephone.

Additional Information

Your starting salary will be pro rata if the above position is part-time or term-time only.
