Description

The Health Visiting Assistant is a member of the Universal Children's Skill mix Team who assists the Health Visitor by undertaking agreed activities and Services for clients that promote health and prevent ill health in both a home and clinic environment.


The post holder will provide comprehensive administration support to the Health Visiting team and ensure that clients are able to make effective use of health promotion materials and information.


MAIN TASKS AND RESPONSIBILITIES:


  • To work as part of the Health Visiting teams, undertaking agreed activities within child health clinics and in the home environment, and reporting back to the Health Visitor in accordance with agreed protocols.
  • To undertake home visits as delegated by the Health Visitor.
  • To undertake open access clinics for parents/carers
  • To assist the Health Visitor in the implementation and evaluation of individual care plans and the maintenance of own RIO progress notes
  • To provide comprehensive support and cover for the Health Visiting Service across the Trust.
  • To support Health Visitors in child health clinics by undertaking growth monitoring and assessment and group work activities.
  • To undertake growth measurement measurements in school as part of the National child measurement programme when required
  • To undertake other relevant duties as delegated by the Health Visitor.


North Mid is part of North Central London integrated care system - consisting of the NHS and Local authority organisations in Camden, Islington, Barnet, Enfield and Haringey.

  • To be familiar with Children's Centres and local programmes to facilitate parents in accessing the Children Centre Community Services and other local initiatives.
  • To be responsible for some coordination and organisation of HVA work and support the development of systems across the Health Visiting teams.
Communication

  • Dealing with sensitive or contentious information
  • To participate in team meetings
  • To assist in developing and maintaining good working relationships with all colleagues, clients and carers.
  • To be conversant with the Trust's Child Protection Procedures, referring any concerns to the Health Visitors.
Patient/Customer Care (if applicable) (both direct and indirect)

  • To undertake home visits as delegated by the Health Visitor
  • To contribute to the development and implementation of programmes of treatment/ care/support for children, young people, families and carers under the guidance of Health Visitor
  • To assist Health Visitor with developmental reviews as required
  • To undertake growth monitoring and assessment in well baby clinics.
  • Undertaking group work activities including the NCMP when required.
  • To contribute to the identification of the Public Health needs of individuals and communities; and working within specified agreed national and local targets.
  • To contribute to the planning, and implementation of effective health promotion and health education programmes for client group within communities to support Public Health Outcomes.
People (HR) Management

  • To participate in local induction of other staff
Information Management

  • Responsible for the maintenance of own record keeping on Patient record information system (RIO) and to adhere to record keeping standards in accordance with Trust guidelines.
