The team requires a Reviewing Officer - Social Care - Qualified (Adults) to be responsible for carrying out strengths-based reviews and reassessments regarding people with long term conditions in accordance with the principles of the Care Act and under the guidance and supervision of the Senior Practitioner.


Key Responsibilities:


  • To visit people in their own homes and also within care home settings to meet with the service users their families and carers to discuss the support plan, which will entail lone working.
  • To draw up an appropriate, budgeted outcome based support plan using the Resource Allocation System and will monitor established plans and residential nursing home placements, ensuring they the support is effective at maximising the person's independence.
  • To provide information, advice and signpost service users to a range of universal services based on the outcome of the assessments and recommend and set up Telecare support where appropriate.
  • To use your knowledge and skills to assess the cost of support appropriate to meeting a service users' needs and be aware of the financial implications of the work undertaken.
  • To be responsible for managing risk, capturing and recording qualitative and quantitative data, raising safeguarding concerns in line with the London procedures and taking responsibility for their own performance to achieve the National Indicator targets for assessments and reviews.

Qualifications

  • NVQ Level 3 or 4 in Social Care
  • Social Work Certificate

Experience

  • Experience of working in a social care setting for adults with long term conditions.
  • Experience of reviewing and reassessing the needs of service users, within a social services context.
  • Experience of carrying out visits to service user's homes and care home settings, to carry out the review.
  • Experience of completing needs led reassessments and reviews from a strengths based perspective.
  • Experience of monitoring and reviewing the impact of support against agreed outcomes to ensure it is effective.
  • Experience of recording confidential and sensitive information accurately and confidentially.
  • Experience of working with service users and a diverse range of people including people from black and ethnic minority communities

Knowledge & skills

  • Knowledge of Adult Social Care
  • Knowledge of the Care Act, the Mental Capacity Act and Safeguarding.
  • Knowledge of current best practice including telecare within Adult Services.
  • Good understanding of the needs of Adults at risk and those with a physical, sensory or learning disability and the impact this has on their ability to communicate and function.
  • Understanding of the issues relating to the delivery of review, reassessment and support planning for people within a diverse community setting and the range of solutions available to facilitate their independence.
