We are looking for an experienced and highly motivated senior manager to help us realise our joint system vision for discharge.

This exciting opportunity is a new post within a newly formed system discharge team and will be key in helping drive the systemwide transformation needed around discharge across all key partners within the Nottinghamshire ICS.

The role will support in the delivery of the requirements of the National Hospital Discharge and Community Support Policy, Operating Model, and the ICS Discharge to Assess strategy with responsibility and accountability to all system partners involved with integrated discharge across the Nottingham & Nottinghamshire ICS.

The post holder will support the System Discharge Lead on the transformation of local services in line with and the ICS Discharge to Assess strategy.

Ensuring continuous improvement and embedding of "Home is Best" approach and contribute to the strategic thinking for the development of proactive and responsive services, maintaining a clear focus on the delivery of high quality, safe, innovative, person-centred services that recognise and promote independence.

In performing this system wide role, the post holder is required to develop a shared system view of discharge, and to hold stakeholders to account, driving forward the actions that need to be taken as a system, to address shared challenges and implement new ways of working.

A key deliverable will be establishing a fully integrated Discharge to Assess (D2A) service and support the on going development of the Transfer of care hubs located within our 3 acute trusts.
