Hours: 37 Hours Per Week (Full Time)


Salary:
£25,000-£26,906 Salary dependent on skills, experience and knowledge.

Base:
Different AMHT/Keystone hubs across Oxfordshire (Travel between sites required) (Hybrid Working options available)

What will you do?


The Adult Mental Health Wellbeing service delivers wellbeing sessions and initial signposting to those who have been supported by Adult Mental Health Teams to provide them with the information and skills to manage their own mental health with mínimal support.

The role is to be embedded with clinical teams, primarily within the Adult Mental Health Teams and Keystone Community Mental Health Framework hubs.

Workers are required to liaise closely with these colleagues to gather referrals, jointly manage risk and identify the best possible onward support for service users.

To do this and promote the service this role also requires attending regular business meetings and other Project Team meetings.


Key responsibilities

  • Deliver an appropriate number of wellbeing sessions to service users referred from secondary services, based on the needs of the individual.
  • Ensure that the service delivered is excellent and person centred.
  • Support individuals to identify their own hopes and goals and develop a personal action plan to increase resilience and maintain wellbeing.
  • Signposting individuals and colleagues to relevant services provided by Oxfordshire Mind and external partners, both mental health specific and widely accessible community provision
  • Deal with referrals and paperwork for AMHT/Keystone referrals according to risk assessments.
  • Ensure timely, accurate reporting on databases and record client feedback through appropriate models.
  • Maintain and develop skills to achieve best practice utilising training and other development opportunities
  • Work within policies and procedures of OXM
  • Be responsible for recording and reporting all safeguarding issues
  • Liaise with partner organisations where required and develop these links
  • Ensure that service users can engage and influence and participate in the running and development of services
