The East Riding Partnership is an established formal partnership between Humber NHS Foundation Trust, The Alcohol and Drug Service (ADS) and The National Association for the Care and Resettlement of Offenders (NACRO).

An exciting opportunity has arisen within the addictions service. We are seeking a Recovery Worker who will take the responsibility of being part of a multi-disciplinary team supporting a complex patient group in their Recovery journey

The post holders will work out of the East Riding Partnership "West Hub" based at Goole.

This will provide a seamless service in a multi-agency and multi-disciplinary approach to substance misuse treatment and support Signposting to appropriate services within the East Riding Partnership, community services and other organisations.


The aim of this role is to help the patients get the right service, at the right time and in the right place.

This may be for screening, health promotion, community detoxification or connecting with the wider community to achieve a meaningful recovery.

You will work with clinical teams within the substance misuse treatment delivery system and role model.

This will include generic assessments, assessing and managing risk, co-working, care planning and reviewing, and assertively working with patients/individuals to support and maintain their recovery.


You will give supporting advice on substance misuse and related issues to patients and liaising with health and social care professionals supporting the recovery outcomes.

This includes advice on treatment options including detoxification, substitute maintenance prescribing, symptomatic management, the management of alcohol withdrawals and detoxification.

You will be supporting and inputting information at the Multi-Disciplinary Team, feeding back to the patient the outcome.

You will also be integral in signposting patients to relevant services and assisting your colleagues to get the best recovery outcomes.

  • To contribute and assist in the process of ensuring carer support is offered and implemented
  • To contribute to and provide information into the care planning, assessment and review process.
  • To provide service users with information on health promotion.
  • To ensure that service users are fully involved and engaged with their agreed care plan, and access appropriate services on a regular and consistent basis e.g., housing, job centre plus, community centres
  • To promote independent living and provide a consistent point of contact for service users and their carers
  • To ensure that the service user understands and has a clear pathway of care across agencies with key contact points/named individuals.
  • To help service users gain access to community resources including benefits and welfare rights.
  • To participate in the implementation of care plans, documenting observations and actions as appropriate
  • To facilitate service users to live ordinary lives by providing regular and practical support to the individual and their carer. e.g., assistance with activities of daily living
  • Be aware of and understand the Clinical Governance and Information
Governance frameworks

  • Carry out the recording and monitoring of Physical Observations and random/ planned drug screening.
