Description

We are actively seeking an experienced Assistant Practitioners who will be focused on supporting people across Edinburgh who are at risk of admission or readmission and to facilitate early discharges from the Acute Medical Unit in the RIE into the community.

Key qualities are a passion for supporting people to improve their quality of life and achieve their goals.

The postholder will require to be proactive, be creative, work as part of a team and demonstrate the ability to tailor interventions as no two days will be the same.

These posts will report to the AHP Team Manager / Home First Manager.

This role will be considered as a Secondment opportunity.

Please note there are two posts offered: 1 WTE and 0.5 WTE
