Role Purpose:


To work within the Short-Term Re-enablement Service, to coordinate and facilitate the safe and effective discharge of patients from Hospital.

Supporting proactive reduction in delayed transfer of care. To provide assessments for both intermediate care services,other discharge pathways as required.

Working proactively within an integrated health and social care teams, to promote formal and informal channels of communication both within the team, CCG and wider community as appropriate.


Key Responsibilities:


  • Maintain own Nursing and Midwifery Council registration and demonstrate commitment to personal professional development.
  • Demonstrate effective communication skills by liaising with multidisciplinary professions, patients, and cares.
  • Contribute to and promote an awareness of audit and research.
  • Participate in service developments in order to assist and support in improving the quality of personcentred care, whilst maintaining professional standards.
  • When necessary, to take responsibility for coordinating the team, appropriately delegating and prioritising workloads and attend capacity meetings.
  • Participate in daily board rounds to coordinate safe, effective discharges of patients and promoting patient flow.
  • Assist in dealing with complaints in the first instance and act as an advocate of patients. Use the correct channels and procedures to refer on complaints that cannot be easily resolved.
  • Effective management of incidents, complaints, and safeguarding.
  • Ensuring compliance with relevant quality policies and procedures and legislative requirements and keeping up to date with changes as determined by the Group Governance team.
  • Ensure you are up to date with CQC compliance requirements, standards, and policies.
  • Ensure all packages meet CQC standards relating to Care Planning, compliance, and contingency.
  • Cooperate with Quality Management team in relation to all aspects of Quality improvement.

Experience and Qualifications required:


  • Registered nurse
  • Degree level qualification relevant to practice or evidence experience of recent CPD
  • Driver's Licence
  • Experience in managing a team
  • Evidence of teaching / supervision skills
  • Computer literate ability to use software programmes designed to maximise the contribution to the post
  • Ability to analyse data and present information to various audiences
  • Demonstrable contribution to practice developments in primary care.
