Description

The Health Care Support Worker has a degree of autonomy and undertakes a broader range of health and care in accordance with policy and procedures.

The post holder will work on their own initiative, undertaking delegated tasks with appropriate supervision in place from the clinical nurse specialist clinical nurse specialist.

The post holder contributes to assessment and assists in the development, implementation and evaluation of individualised care plans for patients with heart failure.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
