Description

We are looking for a dynamic Band 6 nurse to join one of our Community Nursing teams based at Hawkhurst Community Hospital within the Weald PCN.

The service operates seven days a week from 8am - 6pm and these extended working hours allows more flexible working for anyone looking for more diverse working patterns.

Weekend working is generally on a 1:4 ratio with days off during the week.


There are two teams supporting patients registered with GP practices working in the Hawkhurst area and surrounding villages within the Weald Primary Care Network.

Each team has two senior nurses who to take responsibility for a defined geographical area, and provide clinical leadership to their Band 5, Band 4 and Band 3 staff.

The team will work together to manage caseload patients delivering a high-quality service.


Using the named nurse model, we want to allow nurses to spend more time with patients, promoting self-management where possible, and improving patient lives.


Staff will be required to drive their own car and are able to claim mileage costs and access the NHS car lease provision, following their probationary period.

Successful applicants are required to have the qualification level specified in the person specification in order to carry out the job, however, we do welcome applicants from individuals wishing to further their knowledge and skills and to gain this recognised qualification, which we will support.

Monday to Sunday 8am-6pm 1 in 4 Weekends
