Qualified Advanced Clinical Practitioner:
The Frailty Advanced Clinical Practitioner plays a pivotal role in the assessment, management, and care coordination of elderly patients with complex healthcare needs, focusing on frailty and age-related conditions.
This role combines advanced clinical skills, critical thinking, and collaboration to ensure the highest standard of care for this vulnerable population.
The practitioner functions as an autonomous clinician, contributing to the multidisciplinary team to optimise patient outcomes and enhance the quality of care delivered within the healthcare organization.
Trainee Advanced Clinical Practitioner:
The Trainee Frailty Advanced Clinical Practitioner role offers a valuable opportunity for healthcare professionals to develop expertise in assessing, managing, and coordinating care for elderly patients with complex healthcare needs, focusing on frailty and age-related conditions.
This role provides structured training and mentorship, enabling trainees to acquire advanced clinical skills, critical thinking abilities, and collaborative practices within a multidisciplinary team setting.
Trainees will contribute to delivering high-quality care to elderly patients, while progressively assuming greater responsibility under the supervision of experienced clinicians.
ClinicalConduct comprehensive assessments of elderly patients with frailty, cognitive impairment, and multiple comorbidities.
Formulate accurate diagnoses and care plans based on assessment findings and evidence-based guidelines.
Identify and address potential risks and deterioration in patients' health status.
Prescribe and administer appropriate treatments, medications, and interventions within the scope of practice.
Collaborate with all members of the MDT to develop comprehensive care plans.
Monitor patients' progress, adjust treatment plans as needed, and provide education to patients and their families.
Leadership
Take on leadership roles within healthcare teams, guiding and mentoring less experienced colleagues, and contributing to the development of best practices.
Act as an advocate for patients, ensuring their needs and preferences are considered in decision-making, and advocating for high-quality, patient-centered care.
Contribute to the education and training of other healthcare professionals, sharing expertise through mentoring, formal presentations, workshops, and other educational initiatives.
Engage in ongoing learning and professional development to stay updated with the latest advancements in clinical practice and maintain proficiency in the field.
Contribute to research activities, including quality improvement and audit projects and disseminating research findings to improve patient care.
