The post holder will work with the Director of Integrated Community Health Services, the Associate Director of Operations CNWL and the Integrated Care Service Line Management Team to provide strong operational management and leadership for services within the portfolio of Intermediate Care and Adult Community Services.

The post holder is a key member of the Integrated Care Service Line senior management team and will:

  • be responsible for the strategic and operational management of Ealing Community Partners services under the West London NHS portfolio.
- ensure appropriate interfaces are maintained across West London NHS Trust's other service lines hosting mental health services to support the delivery of integrated physical and mental healthcare
- ensure delivery of the required levels of performance; financial balance; and maintain high quality standards across the services
- ensure the successful implementation of relevant transformation plans
- work across the local health and care system, including with external partners, to ensure key interfaces relating to service delivery and effective flow of patient pathways are delivered
- lead on the operational delivery of key metrics and other agreed outcomes to support business as usual, as well as the agreed metrics emerging from service developments
- lead and support clinical staff who provide direct clinical care
- lead on the organisation of on-call arrangements

Strategic and Operational management

  • To be responsible for the strategic and operational management of community services in Integrated Care Service Line as well as ensuring clear links with elements of the Ealing Community Contract delivered in other West London service lines. Elements delivered by our subcontractor CNWL will report into the other ECP Associate Director, but there will be a requirement for close working and, at times, cross cover.
  • To work with the Director of Integrated Community Health Services and others continuously to improve services and to ensure that all aspects of high standards of patient care are adhered to.
  • To provide line management and supervision to managers within the service line and ensure that all staff in the service line receive appropriate supervision including professional and clinical supervision, personal development plans and regular reflective practice. To directly line manage the Heads of Planned and Unplanned Care.
  • To maintain contact with patients and carers of patients using services, as required
  • To maintain a programme of regular engagement, visits, shadowing and visibility across all services including working flexibly across a number of sites and undertaking shadowing and attending team clinical and operational meetings to support understanding of service level issues and positive communications with frontline staff and patients
  • To ensure that clinical governance mechanisms are in place, driven at a local level, ensuring lessons are learnt and experience informs 'best practice'
