Advanced Practitioner - Borough Home Treatment Teams


The Advanced Practitioner will manage staff as delegated by the clinical service lead and provide supervision, expert advice and guidance to members of the MDT including nursing, OT, Social work and medical staff.

Opportunities for shadowing and mentorship for junior staff will be made available.

Trust Home Treatment Teams provide 24hr, 7 day a week, crisis assessment and intensive home treatment to patients with the aim of preventing in
- patient admission where possible or if admission is required facilitating earlier discharge.

Advanced Practitioners undertake assessments and home visits whilst exhibiting the ability to balance both clinical demands and managerial responsibilities.

They will be available shift to shift to consult and take a lead in the treatment of more complex and/or high risk cases demonstrating safe clinical decision-making and expert care, including assessment diagnostic skills and effective interventions.

The post holder will be able to demonstrate critical thinking in the clinical decision making process. They will work with the multi-professional team to meet the needs of the service users and provide clinical professional supervision and leadership to the team


The post holder will provide management support as delegated by the Clinical Services lead, including supervision of staff, team training and service development initiatives.

It is expected that the postholder be responsible for organising the roster ensuing safe staffing at all times. They will also be expected to deputise for the Clinical Service Lead in their absence.


Advanced Practitioners are expected to work either the early or late shift making certain that there is always an Advanced Practitioner on duty during the hours of 8am to 10pm 7 days a week, 365days a year.

The Trust is committed to innovation, effective interventions and improved service user outcomes.

It provides mental health and substance misuse services to the people of Lambeth, Southwark, Lewisham and Croydon, as well as substance misuse services for people in Bexley, Greenwich and Bromley and some more specialist services to people from across the UK.


  • To ensure thorough risk assessment of all service users is line with policy standards
  • To ensure that the team assess, diagnose, plan, implement and evaluate effective interventions for service users presenting with mental health diagnoses and any co morbid conditions, and to be available to advise on the most complex and high risk cases
  • To ensure the team assesses capacity and provides safe treatment and care to those who may lack capacity.
  • Rapidly and continuously evaluates the patient's changing condition and response to therapeutic interventions and modifies the plan of care for optimal patient outcome.
  • Attend high risk or crisis situations to provide management advice and clinical intervention
  • To operate within the legal requirements of the Mental Health Act and be able to exercise independent judgement in the assessment and diagnosis of mental health conditions.
  • To assist the team to effectively prioritise demands on it's resources shift by shift, including rapid response to emergencies.
  • Assess and escalate the need to refer service users presenting with complex long term conditions (e.g. diabetes, asthma,) to appropriate secondary and primary care services as recommended by national and local guidance.
  • To ensure the team promotes and delivers evidence based care for service users with mental health conditions
  • To ensure the team supports service users to adopt recovery strategies that promote their wellbeing, healthy lifestyle, independence and self care.
  • To ensure the team promotes the involvement of families/carers in patient care reflecting their diversity, equality and rights to ensure patients' needs are met.
  • To ensure the team meets the needs of carers, including the offer of specific carer assessment and support
  • Participates in planning, development, and implementation of public and community health programmes alongside the Clinical Service Lead.
  • To ensure that infection control measures are applied within the team's practice according to local and national policies.
  • To ensure safe staffing levels and oversee appropriate lone working practices
