Description

We are currently seeking a Front Door Social Worker Level 2/Care Manager to join our team.

Social Worker within ASC to support on Front Door hub.

The role will be to effectively triage contacts ensuring appropriate signposting.

Also to triage safeguarding contacts and support the decision making process under MSP

Key Responsibilities:
Act as a first point of contact for referrals and assessments.

Conduct initial assessments and screenings.

Determine appropriate support and intervention plans.

Collaborate with multi-agency teams to ensure the well-being of service users.

Provide ongoing care management and support.

Qualifications and Requirements:
Qualified Social Worker (QSW) with relevant certifications.

Previous experience in front-door social work and care management.

Knowledge of relevant legislation and policies.

Strong communication and interpersonal skills.
