Care Coordinators are part of a GP surgery's multidisciplinary team, working alongside GP's, social prescribing link workers (SPLWs), pharmacy staff, nurses, surgery administrative staff etc.

to provide and coordinate, an all-encompassing approach to personalised care and promoting and embedding the personalised care approach across the Primary Care Network.

We are seeking an enthusiastic and committed care coordinator to join our friendly team providing high quality care to our patients.

A pleasant manner, attention to detail, experience of people facing environments are essential. As a care coordinator, the successful applicant will work closely with the clinical and reception team.

The role will include administrative tasks like QOF, EMIS reporting, Nurse Admin support, call and recall of patients for scheduled health checks.

The role involves some patient contact by telephone and Face to Face.

A positive and respectful, and compassionate nature is necessary when delivering a high quality, safe and effective service with the highest standards of confidentiality.

The Care Coordinator will be provided with all the necessary training to implement the project to its full potential.
