An exciting opportunity to help establish and clinically lead the senior mental health practitioners within the living well service in Tameside & Glossop.

This newly established post will involved working with our Pennine Care NHS Foundation colleagues and our Partners in the Living well service as well as external partners to identify and support patients who are struggling with their Mental Health in the community.


This is an interesting and inspiring time to join the neighbourhood mental health team as we are developing together with our partners a service for the residents of Tameside to be able to access support for their Mental health at a time and place that is right for them.

To provide clinical leadership, support and supervision to clinical staff.

and support an identified team of practitioners with advice and guidance on the management of clinical cases and hold a small case load of patients in the 12 week pathway.


  • To work as an integral part of the NMHT
  • To analyze, validate and minimize any internal pathway blockages.
  • To be an effective decisionmaking and clinical resource to support the Team manager in the selections and recruitment of staff, and to develop and promote strategies aimed at staff retention
  • To support the Team manager to complete appraisals for relevant allocated staff within the team.
  • To support the team in developing and building effective communication with all other services.
  • To take appropriate action with regard to complaints and reportable incidents within the service as directed by the Team Manager.
  • To provide clinical leadership and structured clinical supervision to the team.
− Maintain consistency and quality of care for all service
users under the care of the NMHTand support effective
and timely referral on.
− Support safe ,efficient and effective high quality transitions in and
out of the NHMT
− Ensure the team works in a collaborative manner within
the NMHT/LLW with other PCFT services, primary care
and other partners

Job Summary

  • To provide clinical leadership, support and supervision to clinical staff.

  • To participate in the extended working hours as required by the needs of the service
  • To provide clinical leadership and structured clinical supervision to the team.

− Ensure the team works in a collaborative manner within

− Support the team in delivering asset based mental healthcare

  • To ensure that clinical standards are, monitored and
evaluated, and ensure both they and the team provide care
of the highest quality to service users

  • To liaise closely with social care leads, housing
associations, IPS workers and other agencies to promote
social support and aid recovery and promote this way of
working within the team

  • To liaise closely with, general practitioners, to ensure clinical
pathways are supportive of service user flow from CMHT's
to Primary Care and vice versa.
