This post is Fixed Term / Secondment for 2 years to meet the needs of the service.


An exciting and rewarding opportunity has arisen for a suitably qualified, enthusiastic and passionate Health Care Support Workers (HCSW) to join the Bridgend North Primary Care cluster.


We have seven GP surgeries in the cluster, covering a population of approximately 47,000 patients and work collaboratively alongside community pharmacists, district nurses, the third sector and the local authority to support the health of our population.


The 'multi-disciplinary team' approach allows us to meet the health needs of our ageing population and is something we are already fully committed to and working with as a cluster.

We believe this can bring huge benefits to our population with improved access to services closer to home and new and improved services and models of care for the population.


The HCSW will work within the cluster multi-disciplinary team and with the cluster primary care nurses, providing support for individuals presenting with chronic conditions.


The HCSW will also carry out Home Visits on behalf of the registered GP, to include nursing and residential care on behalf of the practices.
