    Job Title: Advanced Clinical Practitioner

    We are seeking a passionate and skilled Advanced Clinical Practitioner (ACP) to join our Ageing Well team at Jurassic Coast Primary Care Network. This role involves providing expert clinical care, focusing on proactive, person-centred support for older adults with complex health needs.

    The successful candidate will conduct comprehensive assessments, undertake reactive visits, manage complex health needs, develop care plans, and prevent hospital admissions. They will also lead on clinical governance, support frailty and complex care programs, and work collaboratively with multidisciplinary teams.

    Main Responsibilities:

    1. Clinical Practice:
      • Independently assess, diagnose, and manage complex cases, including prescribing and treatment plans.
      • Lead comprehensive assessments for complex care needs.
      • Provide expert clinical advice and support across care settings.
      • Ensure continuity of care through effective communication with multidisciplinary teams.
      • Maintain accurate, confidential patient records.
    2. Leadership & Management:
      • Lead clinical supervision and peer reviews.
      • Act as a role model for clinical leadership and safe practice.
      • Support risk management and quality improvement initiatives.
      • Contribute to incident investigations and service improvements.
    3. Education & Workforce Development:
      • Mentor and support staff development through formal education and training.
      • Participate in recruitment and professional development activities.
      • Promote continuous learning and professional growth within the team.
    4. Research & Development:
      • Implement and promote evidence-based practice.
      • Engage in service audits and quality improvement projects.
      • Disseminate findings through professional presentations and publications.
    5. Policy & Service Development:

    Person Specification:

    Qualifications:

    • Registered health practitioner with extensive experience in the relevant specialty.
    • Full Master's degree.
    • Registered non-medical prescriber.
    • Learning and Assessing in Practice qualification or equivalent.

    Experience:

    • Advanced autonomous practice in patient/service user management.
    • Leadership experience, including meeting clinical governance objectives and risk management.
    • Experience supervising and developing multidisciplinary teams, influencing practice changes.
    • Involvement in policy and practice change, including publications, presentations, and research at national/international levels.
    • Development of care programmes, protocols, and audits.
    • Experience in interdisciplinary working and complex care coordination.
    • Managerial experience in clinical supervision and peer caseload reviews.

    Knowledge and Skills:

    • Expertise in clinical assessment, treatment, and evaluation.
    • Advanced skills in assessing complex conditions and decision-making.
    • Highly developed physical skills for precise patient interventions.
