An opportunity has arisen for a motivated, friendly and enthusiastic person to join the Integrated Locality teams as Multidisciplinary Team Lead Co-Ordinator.

The role will be responsible for providing senior administrative support to the locality to ensure smooth running of defined aspects of the administrative function across the locality services, including (where delegated) supervision of the administration assistants.


The Island health and social care services have arranged their teams into 3 geographical areas or localities: The aim is to coordinate care that is provided closer to home, by teams who know and work well together in that neighbourhood.


We are looking for someone who has good interpersonal, communication and customer care skills as the teams are fast-paced and have constant interaction, both face to face and on the telephone with staff and colleagues within the Islands health care system.


The post holder will be responsible for liaising with key contacts within primary care, locality services (including mental health), adult social care and the voluntary sector to co-ordinate multi-disciplinary meetings with a focus on proactive case management of vulnerable people, and for ensuring that all relevant documentation and information is recorded and shared in line with relevant organisations information sharing protocols.

Support the organisation and delivery of the day-to-day administrative functions and duties within the Integrated Locality Services.


Duties will include:


  • Take responsibility for supporting the Integrated Locality administrative functions and the day to day planning and prioritisation of workload to ensure that the locality's needs are met
  • Manage telephone calls some of which could be of a sensitive nature with service users and care professionals
  • Plan, organise and ensure effective running of Integrated Locality MDT meetings
  • Devise and maintain efficient filing systems; clerical and office systems

Job Purpose


The post holder will be the locality point of contact for the co-ordination of the multi-disciplinary case review meetings within the locality and will proactively and positively contribute to the successful overall performance of the integrated locality.


The post holder will be responsible for providing senior administrative support to the locality to ensure smooth running of defined aspects of the administrative function across the locality services, including (where delegated) supervision of the administration assistants.

The post holder will be an active and dynamic part of Integrated Locality Services.
