Key responsibilities Performance Monitoring and Data Analysis Analyse, interpret and evaluate data and monitoring reports, including but not limited to: o Investment and Impact Fund (IIF) o PCN Dashboards o Medicines Optimisation Work Plans o PCN Service Delivery Objectives o Vaccination Services o Enhanced Access o Other PCN held contracts Advise the PCN Board Members and constituent practices on recommended actions to improve delivery against targets, and thereby to maximise fi returns and service quality.

Contribute to implementation of agreed actions as appropriate.

Ensure the PCNs compliance with the GDPR and FOI along with other relevant legislation and professional information management standards such as GMC/BMA and LMC guidance.

Provide performance data for a range of PCN activities as required.

Use Excel spreadsheets to display and predict performance data.

Bring together data from various sources and use information to develop insights into working practices, effectively tailoring content to meet the audience's needs.

Effectively present and communicate data to practices and PCN staff at all levels to increase understanding of operational challenges Interpret data and highlight variation to support open discussion about where variation is and is not warranted Help PCN and practice staff understand the limitations of particular data sets and where other information may be available to supplement understanding of issues Support and encourage a focus on data in understanding the effectiveness of changes made Seek and secure analytical resources to support the PCN in developing and implementing appropriate data and information-sharing mechanisms Use data to track and assess the impact of changes made to operational processes and to track trends and developments over time.

PCN Corporate Governance Support the corporate operation of the PCN, working with the PCN Management Team, Practice Managers and Clinical Director to produce agendas, minutes and reports for Board Meetings, Clinical Workstreams and Operational Working Groups as required.

Ensure completion of PCN contractual documentation and reporting returns.

PCN Development Plan Assist the Clinical Director, PCN management Team, and Practice Managers and PCN Board in the annual review of the PCN Maturity Matrix and PCN Development Plan.

Liaise with practice managers and other staff as required on the implementation of agreed objectives from the PCN Development Plan, including estates planning.

PCN Workstreams Work with the Clinical Director, PCN Management Team, Practice Managers, PCN Board, and member practices to develop and implement work streams to support the delivery of the aims and objectives of the PCN.


Support the Clinical Leads and Project Leads as required to implement PCN pilot projects, pop-up clinics and new services as agreed by the PCN Board.

Work with the Clinical Director, PCN Management Team, Practice Managers, PCN Board, and member practices to develop and manage an overall transformation plan for the PCN, including managing risk, considering the potential impacts on the organisation, and determining resource requirements.

Provide feedback to PCN leadership and member practices on the progress of current initiatives, capacity for change, improvement, and issues so that plans can be adapted in response.

Be the first point of contact for Practice Managers with regard to delivery of PCN projects/schemes/services.

Analyse service specification, performance indicators and data collection requirements to inform design for new PCN services.

PCN Workforce Support the Clinical Director, PCN Management Team, Practice Managers and PCN Operations Manager in recruitment to PCN ARRS roles, including development of job descriptions, advertisements, shortlisting and arranging interviews and follow up processes as required.

Line Manage a team of PCN support staff.

Ensure the ARRS calculator is kept up to date, and work with the Clinical Director and PCN Operations Manager to strategically plan recruitment and associated service development Support the integration of new roles into the PCN and primary care teams.

Ensure the NWRS returns for the PCN are made in a timely manner.

Training & Development Assist the Practice Managers, PCN Management Team and Clinical Director in co-ordination of PCN training and development sessions, including: o PCN Board Development Sessions o Joint Collaborative training o Specific training for new initiatives Communications and Engagement Manage the PCN Website and use of social and digital media, supporting constituent practices as required.

Support the PCN in securing engagement with local community partners In line with the agreed PCN process, establish and support the effective running of the PCN Patient Participation Group.

Support processes to implement patient involvement in service improvement and the wider work of the PCN.
