We are looking for a Physical Health Liaison Worker to join the team in this full-time role in Surrey.

Full-time 37 hours per week


Salary:
£28,000 per annum


Contract: 12 Months Fixed Term (with the possibility of extension depending on funding)

The Role


People living with severe mental illness (SMI) face one of the greatest health inequality gaps in England, dying approximately 20 years earlier than the general population in most part due to preventable physical health diseases.


This new initiative aims to reduce this inequality by supporting the uptake of physical health checks that people with a diagnosed severe mental health illness are entitled to but do not always access.

As a Physical Health Liaison Worker, you will work holistically,directly with individuals with a SMI to increase the uptake of physical health checks and create positive lifestyle changes, in partnership with other key health and social care professionals in the Surrey Heartlands area.


Key responsibilities include:

  • Liaise with primary care staff to identify caseload and support completion of physical health checks
  • Build relationships with caseload of patients to understand and support with any barriers to attending appointments
  • Facilitate appointments for patients on Practices' SMI list to physical checks to be undertaken.
  • Facilitate joint/shared clinics which might be held outside of practises in Community Venues, or provider buildings
  • Appropriately support those who cannot get to appointments
  • Arrange alternative venues if appropriate
  • Explain appropriate activities within the Community Connections service and encourage attendance
  • Provide information about other agencies and organisations that offer specific support for identified areas of concern
  • Bridge people into appropriate external agencies

The charity is a mental health and emotional wellbeing charity, supporting Surrey residents since 1994 in Banstead, Elmbridge, Epsom & Ewell and Mole Valley.


Offering a variety of face-to-face and online services, available to adults (aged 16+) living in Surrey who would like to restore and/or maintain their mental and emotional wellbeing (no diagnosis needed).

The services are free of charge, with no waitinglist and no time limit.
