RT -Social care are working with Tower Hamlet's

  • Paying £30.37 an hour
  • 35 hours per week
  • Working in integrated/partnership within the Neighbourhood Mental Health Team/East London NHS Trust.
  • Provision of a social work service within the framework of the Care Programme Approach & Care Act plus other appropriate legislative requirements.
  • Respond to requests for assistance, taking sufficient basic information about the needs in question to be able to determine the level and type of assessment required as per Care Act and other legislative requirements.
  • Assist users and carers to play a full part in care planning by eReview the progress of each care plan at regular, predeterrmined intervals (within one year minimum) together with users, carers, advocates and other service providers to ensure the effectivenessof the plan to meet agreed outcomes.
  • Carry out responsibilities under mental health legislation and regulations including, where applicable, Approved Mental Health Professional duties.
  • Participate in office duty arrangements and oncall arrangements.
