Remedy Recruitment Group are currently seeking a Team Manager for the Adult's Hospital Integrated Discharge Team.


You will need to be:


  • SWE registered
  • Eligible to work in the UK
  • Ensuring the Team meets statutory and locally agreed obligations to Adults and Older People with presenting Health and Social care needs.
  • Ensuring the Teams work is informed by legislation, statutory and local guidance and current best practice.
  • Continually working to improve the efficiency and effectiveness of the Team and of the Department, contribute to business plans and to policy and practice review and development.
  • Taking a lead role in promoting effective joint working within the department, the Council, external agencies, service users and their carers.
  • Ensuring that all work complies with Council policies and procedures and statutory obligations are met.
  • The supervision and management of a staff team.
  • The practice standards and performance management of the staff team and their adherence to Directorate policies, procedures, statutory responsibilities and agreed performance targets.
  • The quality and prioritisation of your own work, following direction from your Service Manager, and for alerting your Service Manager at the earliest opportunity to any case issues, caseload issues, safeguarding concerns, practice issues, or issues of unaddressedrisk that will adversely impact the health and wellbeing of an individual or their carer, or issues that may impact the good reputation of the department or the local authority.
