To provide safe, personal and effective care and support to patients, families and service users of St Lukes Hospice, Plymouth.

To support the registered nurse and contribute to the ongoing assessment and care planning for patients.

To monitor the condition and health need of patients on a continual basis in partnership with colleagues, families and carers, referring to others for reassessment when required.


The jobholder will contribute to the assessment, planning, implementation, and evaluation of programmes of care as part of the multidisciplinary team.

The jobholder will have the knowledge to interpret information and be skilled in taking appropriate action(s) to achieve the planned outcome.

The post holder will contribute to the maintenance of a safe clinical environment. The post holder is expected to work across a variety of clinical environments.


The job holder will actively support the assessment and supervision of trainee or new nursing associates, assistant practitioners, students and clinical support workers.

This will include leading and co-ordinating a team of clinical support workers.
