Overview:


As a Mentoring Support Specialist, you will provide comprehensive mentoring support to individuals in the Leeds area, both in person and through virtual channels.

This role involves conducting diagnostic discussions, developing action plans, and facilitating interventions to assist customers in achieving their goals.


Main Duties & Responsibilities:

As the post-holder, you will be responsible for the following:

  • Conduct an initial diagnostic discussion and up to three additional interventions based on customer needs. This includes face-to-face delivery.
  • Create SMART action plans with clear pathways and review progress at each intervention.
  • Support customers in attending initial appointments with relevant service providers, as necessary.
  • Engage in case conferencing with JCP staff before and during the provision.
  • Provide regular feedback to work coaches regarding customer progress.
  • Demonstrate a successful track record of working with and supporting individuals with mild to moderate Mental Health issues.
  • Offer support within JCP premises if required and feasible.
  • Provide travel assistance and facilitate engagement with other providers, when needed.
  • Deliver flexible support tailored to the customers' needs, including facetoface, video, and phone interactions.
  • Understand the eligibility criteria for current DWP provision and identify potential next steps for customers.
In addition to the requirements of the above post, you will be expected to:

  • Maintain accurate records and documentation related to mentoring interventions.
  • Collaborate effectively with JCP staff and other stakeholders involved in the customer's journey.
  • Adapt to changing circumstances and customer needs.
  • Comply with confidentiality and data protection regulations.

Essential Criteria:


  • Proven experience in successfully working with and supporting individuals with mild to moderate Mental Health issues.
  • Excellent communication and interpersonal skills.
  • Strong organizational abilities and the ability to manage a caseload effectively.
  • Flexibility to deliver support through various mediums, including facetoface, video, and phone.

Desirable:


  • Knowledge of current DWP provision and eligibility criteria.
