Liverpool First Primary Care Network is looking to employ up to three care coordinators under the Additional Roles Reimbursement Scheme to work across several of its 11 member practices: Abingdon Medical Centre, Benim Medical Centre, Bigham Medical Centre, Dr Jude's Practice Picton, GP Practice Riverside, Hornspit Lane Medical Centre, Rocky Lane Medical Centre, Sandringham Medical Centre, Stopgate Lane Medical Centre, Walton Village Medical Centre and Yew Tree Medical Centre.

Duties of the post-holder:

Undertaking appropriate training required by the Personalised Care Institute.
Working closely and in partnership with Liverpool First PCN's social prescribing link workers.
Utilising population health intelligence to proactively identify and work with cohorts of patients to deliver personalised care.
Supporting patients utilise decision aids in preparation for shared decisionmaking conversation.
Holistically bringing together patients' identified care and support needs.
Exploring options to meet patients personalised care and support plan needs based on what matters to the patient.
Answering queries, making and managing appointments, and ensuring patients have good quality written or verbal information to help them make choices about their care.
Using tools to understand patients' level of knowledge and confidence in skills as regards to managing their own health.
Supporting patients assume training and employment.
Supporting patients to access appropriate benefits where eligible through partnership working with Liverpool First PCN's social prescribing link workers.
Assisting patients access selfmanagement education courses, peer support or interventions that support patients to take more control of their health and wellbeing.
Exploring and assisting patients with personal health budgets where appropriate.
Providing coordination and navigation for patients and their carers across health and care services.
Supporting the coordination and delivering of MDTs within Liverpool First PCN.
Working with GPs and other primary care professionals within Liverpool First PCN to identify and manage a caseload of patients.
Raising awareness within Liverpool First PCN of shared decisionmaking and decision support tools.
Raising awareness of how patients could benefit from shared decisionmaking.
Ensuring referrals, when appropriate, are made to organisations that have basic safeguarding processes in place for vulnerable individuals.
Provide patients with opportunities to develop friendships and belonging, and build knowledge, skills and confidence.
Undertaking monthly supervision with their line manager.
Bringing safeguarding concerns to the attention of a relevant GP and acting in accordance.
