This is an exciting opportunity to join the Cardiff South West Cluster Team in North West Locality delivering a range of health and social care services to the Cluster population.

We are looking for an enthusiastic, committed, customer focused, individual to join a full-time position.


As a Integrated Care Navigator, based in the Cluster Care Hub, your role will be to ensure that a comprehensive administrative and clerical service is delivered and providing excellent service to both staff and patients.


We are looking for professional, patient focused and hardworking individual, with a genuine passion for speaking to patients, eager to learn and keen to deliver in both performance and customer service.

This is an excellent opportunity for a person with interpersonal and organisational skills to work as part of Cardiff South West Cluster team.


We are looking for a reliable person who must be able to work on their own initiative to support a team.

You will be highly organised, proactive, professional and an excellent communicator and be an integral part of the team to provide a high standard of service to colleagues and patients.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

This post is fixed term until May 2025 to meet the needs of the service.


Main duties include:


  • Answering telephones and assisting callers
  • Updating and managing social media accounts & website
  • Create & circulate newsletters
  • Minute taking
  • Creating, circulating meeting agenda's & schedules
  • Manage & action daily discharge advice letters (DALs) list
  • Booking appointments at Surgery for NOAC patients
  • Send post
  • Contacting patients via telephone for wellbeing calls
  • Complete adhoc admin duties within The Hub
  • Provide a courteous telephone service to patients and other users
  • Deal efficiently and appropriately with enquiries received either by telephone, fax or letter from patients and internal and external referrers in a polite, calm and efficient manner
  • Maintain up to date SOPs
  • May be required to work across cluster sites
  • Share responsibility with line manager for own personal


This post will require you to work as part of a team and will require you to have strong IT skills and be confident in undertaking a range of computer-based activities.

The working hours for this post is Monday to Friday 8.30am - 4.30pm
