Job Title:
Domiciliary Care Risk Assessor
Accountable to:
Risk Assessment Manager
Location:
London Borough of Tower Hamlets
Job Purpose:
- Vigorous Risk Assessment to mitigate potential risk to service users and carers.
- To ensure Care Solution Bureau meets Care Quality Commission standards.
- Make sure all clients files are up to date and compliant with Care Quality Commission.
Main Duties & Responsibilities
- All service users must have
person centered care which is holistic and promotes choice and control for the individual. - To provide person centred support to adults within their own home in order to promote independence and personal fulfilment in accordance with company policies and procedures.
- Regularly carryout risk assessments and review individual care plans.
- Assist in coordinating, communication and maintaining relationships with internal and external stakeholders including service users, families, and professionals.
- Provide a responsive, quality service to customers by giving accurate timely information that is understood by service users.
- Carry out risk assessment for all service users.
- Ensure that care plans and records are kept up to date.
- To make sure all service users have a person centered care plan which identifies their needs and promotes health and wellbeing.
- To develop effective service delivery by organising and maintaining administrative provision record.
- To Provide routine information including statistical information
- Assist in coordinating, communication and maintaining relationships with internal and external stakeholders including service users, families and professionals.
- Provide a responsive, quality service to customers by giving accurate timely information that is understood by service users.
- Respond effectively and positively to complaints and breakdown in service delivery.
- Identify shortfall in service delivery and ensure it is dealt with appropriately.
- For monitoring purpose produce quarterly reports, when necessary conduct service user satisfaction survey.
- Visit clients at home to conduct investigations, risk assessments, care plans and provide any support needed at the time.
- Support other senior staff by supervising carers, conducting appraisals, spot checks and auditing.
- Ensure service users have access to the full range of information they need in order to be able to make informed choices about their care and to exercise their rights.
- In conjunction with Coordinator team meet agreed case working standards, monitoring systems/requirements and service goals.
- In conjunction with Care Solution Bureau policy and procedure ensure that service provision works in accordance with and promotes the needs of the diverse needs of the community.
- Provide general advice and guidance to customers to help them to make informed choices
- Participate in ongoing training and development.
- Actively participate in regular supervision and appraisals.

Qualifications/Education
Good standard of English
Good standard of Maths
NVQ Level 2
Experience/Knowledge
Working in a domiciliary care or residential care
Understanding Health and Social Care
Ability to act calmly in emergencies and respond in a professional manner
Ability to work under pressure
Able to communicate in a sensitive and reassuring manner
Ability to communicate at all levels and build effective working relationships with other members of the team
Skills
Good verbal and written communication skills
Understanding and adherence to GDPR policy and procedure
