The Orpington Primary Care Network is a group of general practice surgeries in the outer London suburbs.

We currently have twenty six staff and are looking to expand our care co-ordinator team which currently consists of five care co-ordinators.

Part time Care Co-ordinator sought to work 221⁄2 hours per week on 3 days a week.

A hybrid mix of home and surgery-based working.


Pay rate:
£10.75 per hour.

Permanent contract

You will be providing support for the Orpington PCN wellbeing café, and providing support to the primary care team.

This will involve working with members of the practice to help deliver integrated care, and contributing to fulfilling the requirements of the Network Contract Directed Enhanced Service.

The work will include liaising with patients and healthcare staff, answering queries and ensuring patients are provided with good quality information, running computer searches, identifying cohorts of patients and supporting the general practices to achieve their contract targets.

A minimum of five grade A to C GCSEs including English and Maths required.

Excellent organisational and communication skills, and proficiency in MS Office and web-based services required.
