Within Royal Derby Hospital, you will provide active management to patients requiring IV diuretics within the ambulatory HF unit, review patients that have been admitted to hospital and review patients at home following early supported discharge.

Within Chesterfield Royal hospital, you will provide in reach to the wards and review patients within the community Provide specialist advice, support and education to other health care professionals in the management and treatment of heart failure.
