This is a dual role working full time hours of 40 per week, 24 hours as a family practitioner and 16 hours as outreach practitioner.


Family Practitioner
To engage and support families whose family member is engaged in the residential rehabilitation programme at Abbeycare Scotland.

The role based at Erskine Mains House will involve delivering a bespoke package of intensive, wraparound support for the family and actively support family recovery.


Outreach Practitioner


To provide interventions to clients in preparation for admission to Abbeycare Scotland and for their transition back into the community following their treatment episode at Abbeycare Scotland.

You will work alongside keyworkers, therapists, and the Registered Manager to ensure clients are prepared for each phase of their recovery journey.

As outreach practitioner you will ensure that clients are engaged with the service pre
- and post-treatment in order to facilitate treatment retention and following treatment at Abbeycare Scotland are supported through the transition back into the community to minimise relapse risk, facilitate intervention, develop their recovery and deliver successful outcomes.


Responsibilities:


Family Practitioner (24 hours per week)

  • Engage families and deliver specific specialist support to client's families through the admission, treatment, and post treatment periods.
  • Provide information, practical advice, guidance, and listening/emotional support to families.
  • Act as a key point of contact for support for families through the treatment and recovery process.
  • Develop relationships with family members circle of care such as health or social workers etc where consented and appropriate.
  • Liaise with Abbeycare therapists to facilitate family therapy meetings to improve key family relationships and dynamics.
  • Deliver one to one family interventions and support.
  • Facilitate and develop the Abbeycare family supports group.

Outreach Practitioner (16 hours per week)

Responsibilities:


  • Co-facilitate the prehab group to assist prepare clients for admission to the service.
  • Assist the admissions team with assessments for Prison to Rehab admissions.
  • Liaise in a multidisciplinary manner with keyworkers, therapists, clinical staff, and the registered manager to ensure effective coordination and clients have appropriate and robust aftercare plans.
  • Participate in multi-agency reviews in preparation for discharge as a post treatment contact for the client and other professionals.
  • Contact and engage clients on discharge to monitor their progress in the community and identify any additional support needs.
  • Signpost or refer to additional services in response to identified need.
  • Ensure clients are attending aftercare and follow up on drop out.
  • Facilitate aftercare groups, support groups, recovery activities and support community recovery events.
  • Ensure clients and families access and remain engaged with the Abbeycare family support group.
  • Work dynamically with external partners, sharing information with participating teams, including health and social work services and emergency services such as the police or paramedics where appropriate and intervening to tackle any safeguarding issues.

Salary:
£28,000.00 per year
