We are pleased to present an exceptional opportunity for a skilled Diabetes Assistant Practitioner to join our esteemed Integrated Community Diabetes Service.

In this role, you will play a vital part in providing support and ongoing education to individuals living with diabetes.

This post is a 12-month fixed-term contract or secondment opportunity, with working hours ranging from 30 hours per week to full-time, Monday to Friday with the option of working compressed hours.

Health and Social Care (Assistant Practitioner) FdSc or Health and Social Care Foundation Degree (Fda) or equivalent is essential for applicants.


As the Diabetes Assistant Practitioner, your primary responsibility will involve working alongside our community diabetes multidisciplinary team to deliver holistic care to patients with diabetes while working in collaboration with primary care in achieving the 8-care process.

Your flexibility will be highly valued as you rotate through a variety of clinic locations and conduct home visits within the Heywood, Middleton, Rochdale, and Bury boroughs.

In this role, you will have the privilege of providing comprehensive care, invaluable support, and essential education to patients, empowering them to effectively manage their diabetes.

You will work under the guidance and supervision of highly Specialist Diabetes Nurses/Dietitians.


You will support patients with diabetes who require guidance in relation to the use of technology relating to their diabetes management, this includes the downloading of blood glucose meters, libre/CGM sensors and the use of associated Apps.

You will support the delivery of scripted diabetes education with peer support, review, and training.
